How does the lack of clear and concise communication in your workplace effect your projects and delivery schedule?  
Even the United States Space Shuttle program to suffered two fatal lessons learned that could have been avoided. It appears that in each case the communication of the Challenger and Columbia when concerns were brought to the attention of management to postpone the launch or move forward, the engineers were always told that they didn't have enough data or their data was incomplete or their concerns never made it to the right person who was willing to make the decision to pull the plug on the launch.
